WHAT IS RAPTIVA®? 

Raptiva® is an approved treatment for adults with moderate-to-severe plaque psoriasis that are candidates for systemic (also referred to as whole-body) therapy/phototherapy.   

HOW DOES RAPTIVA® WORK? 

Raptiva® is provided to the patient in the form of a weekly injection. It works by actively suppressing T-cells (those blood-cells that assist in the fight against infection) in the patient’s immune system.

 

When specifically activated, these cells migrate to the skin and can lead to the development of inflammation, in the form of scaly, red patches of skin – which are associated with psoriasis.

By actively suppressing T-cells, however, Raptiva® may decrease the function of the patient’s immune system, which may then increase the patient’s potential susceptibility to infections. 

ARE THERE ANY POSSIBLE HEALTH RELATED RISKS? 

The FDA has reported on three confirmed reports (and one possible report) of a rare brain infection, called Progressive Multifocal Leukoencephalopathy (PML), in patients who had been prescribed Raptiva®.

Three of the patients at issue died.  All four of the patients in question had been treated with Raptiva® for more than three years. None of them were receiving other forms of treatment that might have resulted in suppression of their immune systems. 

Four months earlier, in October 2008, the product labeling for Raptiva® had been revised to highlight – in the form of a boxed warning - the possible risks of life-threatening infections, including PML.

At the time in question, the FDA directed Genentech, the manufacturer of Raptiva®, to develop a risk evaluation and mitigation strategy (REMS) that would include a medication guide to educate patients about the possible risks posed by Raptiva®. 

PML is caused by a virus that specifically affects the central nervous system of the patient. PML generally manifests in patients whose immune systems have been significantly weakened.

 

It may lead to an irreversible decline in neurological function and death.

 

The specific symptoms that may be attributed to PML can include noticeable weakness, coordination-loss, alterations to the patient’s vision, problems in speaking, and changes to mood and personality.

Currently, there is no known, effective prevention or treatment for PML. 

Today, the FDA strongly recommends that all health-care professionals very carefully monitor those patients who have been prescribed Raptiva® - as well as those who have discontinued usage of the drug - for any potential signs or symptoms of neurologic disease, such as PML.  

The FDA also recommends that health-care professionals periodically reassess the benefits of continued treatment.  

Patients themselves should be fully conversant with the symptoms of PML, and are advised to contact their health-care professionals immediately, in the event that they experience any symptoms that may be attributed to PML.

  • Progressive Multifocal Leukoencephalopathy (PML)
