WHAT IS HRT? 

HRT is a system of medical treatment specifically designed for women who are (a) surgically menopausal; (b) perimenopausal (defined as the phase before menopause occurs); or (c) to a lesser extent postmenopausal. HRT may prevent and/or lessen the discomfort that results from diminishing circulating estrogen and progesterone hormones in the body of the patient.  

HOW DOES HRT WORK? 

HRT involves the use of one or more of a group of medications designed to artificially boost hormone levels. The key types of hormones involved are estrogens, progesterone or progestins, and sometimes testosterone. 

ARE THERE ANY POSSIBLE HEALTH RELATED RISKS? 

Hormone replacement therapy can cause a wide range of distressing, none-life-threatening side effects, including:

  • Headache
  • Upset stomach
  • Stomach cramps or bloating
  • Diarrhea
  • Appetite and weight changes
  • Changes in sex drive or performance
  • Nervousness
  • Brown or black patches on the skin
  • Acne
  • Swelling of hands, feet, or lower legs due to fluid retention

However, far more serious side-effects have been reported in women taking HRT. 

Attitudes concerning the widespread use of HRT - on the part of both the medical community and patients prescribed HRT - shifted drastically in 2002 when an announcement was made by the Women's Health Initiative of the National Institutes of Health that the use of a particular treatment which was a major part of their study coincided with a larger incidence of breast cancer, heart attacks and strokes.  

The treatment in question was called Prempro. Used to treat such menopausal symptoms as hot-flashes, thinning of the bones, and vaginal burning, irritation, and dryness, Prempro is comprised of a combination of conjugated estrogens and medroxyprogesterone.  

Conjugated estrogens are a mixture of estrogen hormones; Medroxyprogesterone is an important factor in the regulation of both ovulation and menstruation. 

The statistical conclusions of the WHI were stark, and revealed that estrogen-progestin HRT treatment had led to an alarming increase in potentially fatal side-effects:

  • A 29-per-cent increase in heart disease
  • A 26-per-cent rise in breast cancer
  • A 41-per-cent increase in strokes
  • A doubling of the risk of developing blood-clots

Those same conclusions and findings of the WHI were bolstered by the results of a large-scale, nationwide study undertaken in Britain called the Million Women Study.

As a result of the collective findings, the incidence of women taking hormone treatment fell by nearly fifty per cent.  

A warning that followed the announcements – and which was published in the Journal of the American Medical Association and elsewhere - stated that women with normal, rather than surgical, menopause should only take prescribed HRT treatment at the lowest feasible dose, and for the absolute shortest period of time considered permissible.  

In March 2006, the Journal of Cancer stated that post-menopausal women prescribed HRT had an elevated risk of developing breast-cancer.
