WHAT IS PPH? 

Primary pulmonary hypertension is defined as an increase in blood pressure in the patient’s pulmonary artery, pulmonary vein, or pulmonary capillaries.

PPH can result in a variety of symptoms and conditions, including shortness of breath, extreme fatigue, angina, swelling of the ankles and the feet, the occasional coughing up of blood, dizziness, and fainting – all of which may be made much worse by physical exertion.

Most serious of all, PPH can ultimately result in heart failure and/or death.  

PPH has been reported in numerous people prescribed the drug Fen-phen®. 

WHAT IS FEN-PHEN®?

Fen-phen® was a medication designed to treat obesity, and which consisted of two particular drugs: fenfluramine and phentermine.  

In July 1997, a research team based at the Mayo Clinic and Mayo Foundation reported no less than 24 cases of rare valvular disease and PPH in women who had taken Fen-phen®.

In addition, the Food and Drug Administration (FDA) alerted medical doctors to the fact that it had received an additional 9 reports of the same type, and advised all health care professionals to report any and all such cases to the agency’s MedWatch program, and/or to their respective pharmaceutical manufacturers. 

The FDA’s decision was based upon the findings of doctors who - via the use of echocardiograms designed to evaluate the function of heart valves - had studied a variety of patients taking the fenfluramine and phentermine-based product for obesity.

As a result of this study, a disturbing discovery was made. The collective findings revealed that approximately 30 pe rcent of all the evaluated patients had distinctly abnormal echocardiograms, even though they displayed no actual, physical symptoms.

Significantly, this percentage of abnormal test results far exceeded that which could be expected from a comparatively-sized sample of the population who had not been exposed to either fenfluramine or dexfenfluramine, via Fen-phen®. 

In the wake of this action, the FDA received 66 additional accounts of heart valve disease, all of which were associated with the use of Fen-phen®. In addition, documented reports surfaced that demonstrated acute heart-valve problems in patients taking only either fenfluramine or dexfenfluramine.  

The overall outcome was that in September 1997, the FDA requested the withdrawal of Fen-phen® from the U.S. market. Today, Fen-phen® is no longer generally available.

A study published in November 2008 in BMC Medicine demonstrated that patients who had taken Fen-Phen® could possibly be affected by heart-related problems for years after being prescribed the drug.
